To Print Your Medical Release Form

To print out a Medical Release form that virtually just needs a signature, go to your

CYT home page.

If you have already registered for classes click "view registration". There is a column

on the left titled, First Class Day, click on "Medical Release" and then "Behavior Contract".

Print out the Medical Release form. You may also print out the behavior contract

for reference. You and your student must read the behavior contract and sign at the bottom of the Medical Release form indicating your understanding and cooperation.

Print out two Medical Release forms if you plan to audition as one is needed at

the first class day and at auditions!
